Details about how Philips New Zealand Commercial Limited (CN 5716235) (“Philips”) will use your personal information (including any meta-data to the extent it is personal information) collected from you and/or from the device you returned to Philips is set out below. By providing your information to Philips, you are agreeing to this process. If you are under 18 years old, please ensure a parent or legal guardian responds on your behalf.

Your personal information will be used by Philips, its affiliates, its trusted contractors and advisers, and disclosed to regulatory authorities, your health care provider/ care team, your carer or nominated representative, for the purposes of this Product Defect Correction, for Philips product quality management, for investigating and responding to problems, complaints or claims brought by patients or third parties, device analysis and examination (including analyzing device usage data) and to meet Philips’ legal obligations.

Your personal information will be securely stored on Philips' computer systems. Philips’ computer systems and database are held on secure third party computer systems located outside of New Zealand, including but not limited to Australia and Singapore. 

The information collected and stored by Philips may be used, disclosed and stored in accordance with this privacy notice, Philips' privacy policy and the Privacy Act 2020 (NZ).   

You can decline to have your information collected or used, but this may mean that the services cannot be provided to you.
